FSN assists families in obtaining large items such as wheelchairs, van lifts, room
remodeling and other specialized equipment that they cannot afford. Items come directly
from the Change A Life Foundation and must range between $500 and $7,500. Clients
need to be referred to FSN for this program and must qualify financially to be eligible by
the Change A Life Foundation. Grant cycles occur five times a year.
Core Requirements:
1. Catastrophic life event occurred (disability, accident, illness, domestic violence, etc)
2. No fault of the individual
3. Grant will create real life change
4. Grant is for a critical need
5. Must meet HUD low income guidelines
6. Last resort request
7. One-time assistance
8. Request is for short-term intervention
